On 17 June 2026, the SMART & STRONG Project was presented at the 3rd World Congress of Age-Friendly Cities and Communities (WCAFCC), held in San Sebastián, Spain (June 16-18).
The project was featured in the parallel session entitled “Building More Sustainable Age-Friendly Communities: Focusing on the Sustainability of Age-Friendly Initiatives.”
The presentation was delivered under the title:
“From Local Innovation to Cross-Border Mutual Learning: The SMART & STRONG Project.”
Overview of the Congress
The Congress is a global forum associated with the WHO Global Network for Age-Friendly Cities and Communities (GNAFCC), bringing together policymakers, practitioners, researchers, and community stakeholders to share policies, practices, and research aimed at addressing population ageing and promoting healthy ageing.
The 2026 Congress was held under the overarching theme:
“Embracing the Present, Preparing for the Future.”
The discussions were guided by four key concepts:
- Inclusion
- Exchange
- Collaboration
- Emerging Perspectives
Local governments, academic institutions, international organizations, and civil society organizations from around the world gathered to discuss the future of age-friendly communities.
The Congress explored ageing not only as a health and welfare issue, but also from broader perspectives including community development, citizen participation, human rights, and sustainable development.
Presentation Highlights
The SMART & STRONG Project is a cross-border mutual learning network built upon the STRONG Model, a community-based integrated care model developed in Bueng Yitho Municipality, Pathum Thani Province, Thailand. The project is jointly promoted by local governments, universities, NGOs, and other partners in Thailand and Japan.
The presentation introduced:
- Community-based integrated care led by local residents;
- An integrated support system combining health, medical care, welfare, and social participation;
- A low-cost model that can be sustained within local government budgets;
- Mutual learning among municipalities;
- An equal partnership between Thailand and Japan.
The network, which began with nine municipalities in 2022, has expanded to thirty-nine municipalities by 2026 and is attracting growing interest as a locally driven approach to addressing the challenges of ageing societies.
Key Takeaways & Way Forward
One of the strongest impressions from the Congress was the growing global recognition of the importance of community-led action.
Throughout discussions on the post-SDG agenda, the importance of locally led initiatives and equal partnerships is repeatedly emphasized. Similarly, discussions surrounding the proposed Convention on the Rights of Older Persons have highlighted the importance of ensuring people's right to continue living in familiar communities while maintaining their dignity and quality of life.
The Congress reinforced our belief that the SMART & STRONG Project may represent one possible pathway toward more sustainable and community-led responses to population ageing.
At the same time, neither Japan nor Thailand holds all the answers.
The Congress provided opportunities to learn from innovative practices and experiences from around the world. There is no single solution to population ageing or community development; each community is exploring its own path forward through continuous learning and adaptation.
The SMART & STRONG Project will build on the knowledge, insights, and networks gained through this Congress and continue working with partners around the world to contribute to the creation of more sustainable and inclusive communities through mutual learning and collaboration.

本プロジェクトは、分科会「Building More Sustainable Age-Friendly Communities: Focusing on the Sustainability of Age-Friendly Initiatives（より持続可能な高齢者にやさしいまちづくり ― 高齢者施策の持続可能性に焦点を当てて）」において、
「From Local Innovation to Cross-Border Mutual Learning: The SMART & STRONG Project（地域発イノベーションから国境を越えた相互学習へ ― SMART & STRONGプロジェクト）」
と題して紹介されました。
会議の概要
本会合は、WHO（世界保健機関）の「高齢者にやさしい都市・地域グローバルネットワーク（GNAFCC）」に関連する国際会議であり、高齢化社会への対応や健康長寿の実現に向けた政策・実践・研究を共有する世界的なフォーラムです。
今回の会議は、
「Embracing the Present, Preparing for the Future（現在を受け入れ、未来に備える）」
を総合テーマとして開催され、
- Inclusion（包摂）
- Exchange（交流）
- Collaboration（協働）
- Emerging Perspectives（新たな視点）
をキーワードに、自治体、研究機関、国際機関、市民社会組織などが集い、高齢者にやさしい地域づくりの将来像について議論が行われました。
また、高齢化への対応を福祉・医療の課題として捉えるだけではなく、まちづくりや住民参加、人権、持続可能な開発といった幅広い観点から議論が進められました。
発表の概要
SMART & STRONGプロジェクトは、タイ国パトムタニ県ブンイトー市で発展した地域包括ケアモデル「STRONGモデル」を基盤に、タイと日本の自治体、大学、NGO等が連携して推進している国境を越えた学び合いネットワークです。
発表では、
- 地域住民が主体となる地域包括ケアの仕組み
- 保健・医療・福祉・社会参加を統合した支援体制
- 自治体予算の範囲内で継続可能な低コストモデル
- 自治体間の学び合い
- タイと日本による対等なパートナーシップ
について紹介しました。
2022年に9自治体から開始したネットワークは、2026年には39自治体へ拡大しており、地域主導による高齢社会への対応モデルとして国内外から関心を集めています。
世界の議論は「地域の力」へ
今回の会議を通じて強く感じられたのは、世界の議論が「地域の力」に向かっていることでした。
ポストSDGsを見据えた議論では、地域主導の取組や対等なパートナーシップの重要性が繰り返し強調されています。また、高齢者権利条約をめぐる議論においても、「住み慣れた地域で、その人らしく暮らし続ける権利」が重要なテーマとして取り上げられてきました。
そのような世界的な潮流の中で、自治体、住民、大学、医療・福祉関係者が国境を越えて学び合いながら地域包括ケアを育ててきたSMART & STRONGプロジェクトは、これからの時代に求められる一つのモデルとなる可能性を持っていることが改めて実感されました。
一方で、その答えを日本やタイだけが持っているわけではありません。
会議では世界各地の実践事例や創意工夫に触れ、多くの刺激を受けました。高齢化への対応や地域づくりに唯一の正解はなく、それぞれの地域が試行錯誤を重ねながら未来を模索しています。
SMART & STRONGプロジェクトは、今回の会議を通じて得られた知見やネットワークを今後の活動に生かし、世界各地の仲間と学び合いながら、より持続可能で包摂的な地域社会づくりに貢献していく所存です。
